I'm going to Japan in April with a few friends and we've been planning on going to a mixed-gender onsen. I did some research and many onsen review sites and reviews on TripAdvisor say that only women are permitted to wear towels while men are required to be naked. Is this true?

Nah, that’s not true. Men get small towels as well. At least to the ones I’ve been, which is a mix of two high end resort ones in Japan, and various mid range ones Japan/Korea. Honestly you’ll probably just get over it though pretty quick. It’s not about being perverted it just kills the vibe to be worried about covering your junk. No one is there to peep at you, and it’s fairly likely depending on where you go that the mixed pool you go to will either be empty when you go or only men.
It just struck me you might be expecting some kind of anime sex thing. It’s really not like that at all. It’s a great experience but you’re gonna need to go to one of the ones that rents out hookers if you’re looking for something out of your AVs

In 2024 a lot of places don’t care. Sento for example are a bit different from onsen and obviously lower market but no one cares at all there anymore. Even higher market onsen are following suit

You'll probably want to get your own private onsens if you're a trans. Japan aren't very accepting of trans people and I've seen first hand a trans male get laughed at and kicked out of a male onsen.

I am not sure how the Japanese are irl with their culture role of 'a male who looks very feminine and wears female clothes but still says they are male because it is factual' which is how my friend and I tend to act, but we figure the private onsen is a nicer experience anyways and easier all around. She speaks Japanese reasonably.
