This isn't the 80s anymore. Been to Catania and Taormina twice for over two weeks each, it's kinda safe. Just look out for stuff like pickpocketing and such.
Mafia stopped doing murders and homicides after the 90s
Now all their crime is business related (drug, prostitution, construction) and they understood that the more they keep a low profile, the less the government and the general public notice its presence.
Now, theres always the possibility you'll be mowed down by a terroni driving his Fiat Panda on your face while burning a red light going against traffic in the streets of Palermo. That's very likely to happen
Honestly it’s a million times safer than wherever you’re from. I’m a quasimodo tier uggly manlet and I would actually get beaten and stabbed sometimes just for going to the movies on friday/saturday night. I moved to a different country, and while I still get refused service sometimes by women, everyone mostly treats me normally or even with pity. That means doors held open for me, people asking how I’m feeling, etc.
Best decision I ever made was to move. It also cured my IBS

Outside of Naples you aren't going to get robbed... Even Naples is probably safer than any burgerland you've been to.
I did hear of a couple backpackers getting mugged in Palermo while I was there. I went out by myself to a street party and got nearly black out drunk and walked back alone to my hotel at 3am and I was fine though and never felt weird.

You'll be fine. All the crime happens in the major cities full of tourists, arabs, africans, and gypsies
Even then it's still safer than all of america and most of the bigger northern european cities
