He was born in Guildford, NSW on December 27, 1944 to Stjepan Milat, a Croatian immigrant, and Elizabeth Piddleston, an Anglo Australian. Ivan had 13 siblings. The Milats were generally responsible parents who tried to care for their large brood properly although with so many mouths to feed, they lived in comparative poverty, residing in a rural cottage farm in Bosley Park and later moved to Liverpool. Stjepan was sometimes hot-tempered and angry when he had too much to drink. Ivan and his ten brothers were rambunctious boys who enjoyed playing with knives and firearms and were often reprimanded by police for one reason or another.

From an early age, Ivan displayed sociopathic behavior such as attacking animals with machetes. At 13 he was put in a residential school. At 17 he spent time in juvenile detention for theft and at 19 he broke into a store. Shortly before his 20th birthday, Ivan received 18 months in jail for breaking and entering. A few weeks after his release, he stole a car and was sentenced to two years of hard labor. In September 1967 he received three years for larceny.

    Milat's behavior took on a more sinister dimension on April 7, 1971 when he kidnapped a pair of 18 year old girls hitchhiking near the Liverpool rail station at knifepoint. One of them was raped before they stopped at a cafe, where they both escaped. Milat was arrested and charged with rape and armed robbery. While awaiting trial, he and two of his brothers committed a robbery spree before faking his own death by leaving his shoes at The Gap, a cliff overlooking the Tasman Sea that was often known for hosting suicide jumpers. It was believed that Milat went to Queensland and Victoria where he boarded a plane to New Zealand and hid out there for two years. He is thought to have returned to Australia with a fake passport and lived outside NSW to avoid detection.

    Milat was apprehended in 1974 but the outstanding charges against him were defeated in court by celebrity Australian attorney John Marsden. He began working as a truck driver the following year and was gainfully employed in this job for much of the next two decades. In 1977, Milat abducted and attacked two hitchhikers between Liverpool and Canberra; they both escaped and no charges were filed.

    He got married in 1984 at the age of 39 to a 16 year old girl named Karen Duck, who had been knocked up by his cousin. They later had a daughter of their own but Karen left him after only three years due to his violent behavior. At the trial, she said Milat was a gun nut and remembered him shooting kangaroos on a visit to Belanglo State Forest.

    Several young hitchhikers vanished between 1989 and 1992, including native Australians, British, and German tourists. On September 19, 1992, two joggers found a body in the Belanglo State Forest. The next morning, police found another body 100 feet from the first. These were confirmed from dental records to be the remains of Caroline Clark and Joanne Walters, ages 21 and 22, who vanished on April 18. Walters was stabbed 15 times, including nine thrusts to the spine which would have left her paralyzed. Clarke was shot ten tiimes in the head.

    On October 5, 1993, a local man looking for firewood found human bones in a remote area of the forst. Police were summoned and the remains would be identified as those of Deborah Everist and James Gibson, both 19 and residents of Frankson, who disappeared December 30, 1989. Gibson's skeleton was in a fetal position and had eight stab wounds. A knife thrust to the upper spine would have paralyzed him and other thrusts to the back and chest would have gone into his heart and lungs. Everist was brutally beaten; her skull had two fractures, her jaw was broken, and her forehead had knife marks. She'd been stabbed once in the back. It seemed strange that Gibson would turn up in the forest as his camera was found the day after he vanished and his backpack on March 13, 1990 by the side of the road at Galston Gorge, 75 miles away.

    On November 1, a skeleton was found in a clearing along a fire trail in Belango which was identified as the remains of Simone Schmidl, a 21 year old German tourist who disappeared January 20, 1991. She had also been stabbed in the spine and chest. Clothing found nearby was not Schmidl's but those of another German tourist, 20 year old Anja Habschied. Her remains and those of her boyfriend 21 year old Gabor Neugebauer, were found on November 4 in shallow graves 160 feet apart. Habschied was beheaded and her skull nowhere to be found. Neugebauer was shot in the head six times.

On October 14, the NSW Police set up a task force and a bounty of 500,000 Australian dollars was offered for information leading to the capture of whoever did this. It was thought that several different men could have done the killings due to the nature of the crimes.

      On November 13, NSW police received a phone call from the UK. A 24 year old British man named Paul Onions said he was in Australia back in January 1990. He'd been backpacking and hitchhiking from Liverpool station to Mildura on January 25, took a ride south out of Casula from a man he only knew as "Bill", and near Belanglo, Bill pulled out a revolver and rope which he tried to use to rob Onions, who escaped as Bill shot at him. Onions flagged down a motorist named Joanne Berry and the two reported Bill to Bowral police. On April 13, 1994, detectives re-found the note regarding Onions' call. His statement was corroborated by Berry, along with the girlfriend of a man who worked with Milat.

      Police began watching Milat's home on Cinnabar Street in Eagle Vale on February 26, 1994. They found that he'd recently sold his Nissan Patrol after the bodies of Clarke and Walters were discovered and it was found that he'd not been at work the day the attacks happened. Acquaintances noted Milat's fascination with weapons. Paul Onions flew to Australia to assist in the investigation and on May 5 he identified Milat as the man who tried to abduct him.

    Milat was arrested at his home on May 22 for robbery and weapons charges. Inside was a ,22 Anschutz M1441 rifle and parts of a .22 Ruger that was linked via ballistics testing to the murders, a Browning pistol, and a Bowie knife. Items belonging to the victims were found in Milat's house, his mother's house, and five of his brothers' places. Milat himself appeared in court on May 23 but did not enter a plea. On May 31 he was charged with the seven backpacker murders. His brothers Dick and Walter were charged with drug possession, possessing illegal firearms, and possessing stolen property.

    His trial in the NSW Supreme Court began March 26, 1996. Although the evidence against Milat was overwhelming, he was remarkably smug and self-assured--as he would later explain, "My basic defense in my trial was that it wasn't me. I don't know who did it. It was up to them to prove my guilt, not for me to prove my innocence." He suggested that one of his brothers was actually the guilty party.

    On July 27, after an 18 week trial, Milat was found guilty of all seven backpacker murders as well as the attack on Onions and sentenced to life in prison. On his first day at Mailtland Gaol, Milat was assaulted by another inmate. On May 16, 1997, he tried to escape with George Savaas, a convicted drug dealer and ex-Sydney councillor. The escape failed and Savaas hanged himself in his cell. Milat was moved to the maximum security Goulburn Correctional Center. The following November, he appealed on the grounds that he'd been denied his right to legal representation but the NSW Court of Criminal Appeal rejected it. In 2004, Milat appealed to the High Court of Australia that he be allowed special leave to appeal on new grounds. The application for leave was ultimately dismissed, affirming the Court of Criminal Appeal's decision to disallow his initial appeal. Continued appeals over the next several years were also turned down.

    In 2001, he swallowed several sharp metal objects. In 2006, Milat earned the privilege of getting a TV set and a toaster in his cell which caused a public outcry. On January 26, 2009, he cut off his pinky finger with a plastic knife so he could mail it to the HCoA to force an appeal. He was taken to Goulburn Base Hospital but doctors decided the finger couldn't be reattached. In May 2011, Milat went on a nine day hunger strike to demand he get a PlayStation 3 in his cell; this ploy was unsuccessful and cost him 55 pounds of weight.

    In May 2019, he was transferred to Prince of Wales Hospital in Randwick and was diagnosed with terminal esophageal cancer. After being treated, Milat was sent to Long Bay Correctional Centre. He was brought back to the hospital on August 9, 2019 ill with a fever and weight loss but his condition was reported as stable. On October 27, Milat died of esophageal and stomach cancer. He was 74. Milat's remains were cremated. To his last day of life he would not admit to any of the backpack murders despite various attempts by police to get a confession out of him. Although Milat refused to admit his guilt to the authorities, he reportedly told his mother that he did the murders. He may have killed more over the years; at least two dozen unsolved murders and disappearances in Australia over the 1970s-80s may have been done by him.

    Three victims are thought to have had a strong likelihood of being killed by Milat. These included:

    *Keren Rowland, who was pregnant at the time, disappeared February 26, 1971 near Canberra. Her remains were found on a pine plantation on May 3 but no cause of death was determined; Milat's co-workers said he'd bragged about having committed a murder when she went missing and his job was known to take him through this area.
    *An 18 year old man named Peter Letcher disappeared November 13, 1987 near Bathurst and his remains were found near the Jenolan Caves tourist site on January 21, 1988. He was handcuffed, shot in the head with a .22 caliber gun, stabbed, and possibly raped.
    *On September 6, 1991, a 29 year old woman named Dianne Pennacchio was traveling to Lake George Hotel in Bungendore and tried to hitchhike home. Her body was found November 13 in the Tallaganda State Forest clad in only underwear and pants, a stab wound to the back which would cause paralysis. It seemed as if rape had been intended.
